موجز: Summary: يُستخدم هذا النموذج لتقديم طلب اعتماد للتكامل مع بوابة الإصدار/واجهات التحقق/الشهادات أو لتقديم خدمات وسيطة مرتبطة بـ UICS/ICN. Use this form to apply for accreditation to integrate with Issuance/Verification/Certificates or to operate as an intermediary related to UICS/ICN.

  • Compliance with Acceptable Use and Information Security policies.
  • Transparent verification endpoints and active verification links for badges/certificates.
  • Data protection controls and data minimization commitment.

Verification Provider Identity Broker Merchant / Platform Auditor / Assessor Training / Education Partner Other Intermediary

References or relevant technical/security certifications may be required.

4) التحقق والمراجعة4) Verification & Review

  • استلام وإصدار رقم ملف خلال ≤ 5 أيام عمل.
  • مراجعة فنية/أمنية ومحاذاة استخدام العلامة.
  • اختبارات Sandbox قبل أي وصول إنتاجي.
  • Intake and case ID within ≤ 5 business days.
  • Technical/Security review and brand usage alignment.
  • Sandbox tests required prior to any production access.

5) الرسوم والتجديد5) Fees & Renewal

قد تُطبق رسوم اعتماد/اختبارات وفق سياسة الرسوم والاشتراكات. صلاحية الاعتماد سنوية قابلة للتجديد بعد مراجعة مختصرة.

Accreditation/test fees may apply per the Fees & Subscriptions Policy. Accreditation is annual and renewable after a short review.

6) الخصوصية والاحتفاظ6) Privacy & Retention

تُستخدم بيانات الطلب للمراجعة والاتصال والتدقيق، وتُحفظ وفق سياسة الاحتفاظ والحذف و سياسة الخصوصية.

Application data is used for review, contact, and audit, retained per the Retention & Deletion Policy and Privacy Policy.
